Interested in playing a sport at YMS?
We have chosen to use a web-based program for participation in sports here at Yearling Middle School. Below or on our Yearling Middle School website https://yms.okee.k12.fl.us/ are what you need to get started.
- First go to the website: www.finalforms.com (Click this link for a quick instructional video)
- Create an account. (you will need to have an active non-school email account)
- The program will ask both the student and the parent/guardian to complete information step by step.
- Next go the website: www.nfhslearn.com (Click this link for a quick instructional video)
- Create an account. (you will need to have an active non-school email account)
- Complete courses below and upload certificate number into finalforms.com
- Schedule and have doctor complete FHSAA Physical. Make a copy of the physical for yourself and then turn the original into the office.
**Pediatricians can oftentimes take weeks to get appointments scheduled but are usually free. Chiropractor/Urgent Care appointments can sometimes be scheduled and completed same day but costs $25-$50.
